New INTERN Instructions
Congratulations on accepting an internship with People Incorporated! As you may know, we "promote and secure independence for people with mental illness" and soon you will join us in putting our clients first. Before you are able to start at your program there are forms and trainings to complete. Human Resources will partner with you to assemble all of your paperwork prior to your start date. PLEASE NOTE ALL INTERNSHIP PAPERWORK MAY BE SUBMITTED TO HUMAN RESOURCES THROUGH THE FOLLOWING: EMAIL: employment@peopleincorporated.org (file size limit 15MB or 15000KB per email) FAX: 651-454-5050 FIRST: Complete and submit your Background Study Authorization forms TIME SENSITIVE DUE Fridays at 12:00pm for next Friday Orientation You will receive Background Study Authorization forms attached to your Welcome letter. Complete and sign the forms and submit to Human Resources. SECOND: Schedule your Tuberculosis (TB) test TIME SENSITIVE TB RESULTS DUE Thursdays at 12:00pm for Friday New Hire Orientation Contact one of our clinics to schedule your test. Bring the TB Test Form with you and submit the completed results to Human Resources. You must let the clinic know you are an employee at People Incorporated and they will bill us directly. THIRD: Schedule participation in Orientation Orientation is held weekly on FRIDAYS with the exception of holidays, from 9:00 a.m. - 11:00 a.m. at the Administration Office. Email employment@peopleincorporated.org to confirm a date. FOURTH: Complete the required trainings and knowledge assessments TIME SENSITIVE DUE Tuesdays at 5:00pm for Friday Orientation •· Complete the HIPAA training and Knowledge Assessment with a score of 70% or better. Sign in as a guest for the Knowledge Assessment. Print or save the score page and submit it to Human Resources. https://hipaacourses.dhs.state.mn.us/privacy/index.htm •· Complete the Vulnerable Adult/Mandated Reporter training and assessment. Submit the certificate of completion to Human Resources. http://registrations.dhs.state.mn.us/WebManRpt/ •· Complete the Fraud, Waste, and Abuse training and assessment. Submit the signed attestation to Human Resources. FIFTH: Complete the following identification processes TIME SENSITIVE DUE Tuesdays at 5:00pm for Friday Orientation - Bring one of the following to your Orientation:
•· Driver's License •· Passport •· Passport Card •· State ID Card
• Review the Hepatitis B Fact Sheet and then complete and submit the consent form. • Hepatitis B Consent Form Provide us with a copy of your current transcript. - SEXUAL MISCONDUCT BACKGROUND STUDY
If you have worked in a role where you provided direct client services in the last five years we require that you complete one SEXUAL EXPLOITATION BACKGROUND CHECK FORM for each employer where you worked in that capacity. Submit the form(s) to People Incorporated and we will mail them out and track receipt.
• If you will be an intern in the Case Management Division, we require: •· A copy of your valid driver's license •· Proof of current auto insurance.
