Congratulations on accepting a new position and starting a new adventure!
Before you can attend New Hire Orientation you must turn in ALL pre-employment paperwork listed below. Human Resources will partner with you to assemble all of your paperwork prior to your start date and offers you this CHECKLIST to help you stay organized.
Paperwork may be submitted to Human Resources through the following:

Ø FIRST: If you are professionally licensed, please complete this form and submit it to Human Resources immediately! We will confirm your license and begin the credentialing process if it is applicable for your position.
Ø SECOND: Begin the 1st step of the Tuberculosis (TB) testing process
TIME SENSITIVE TB RESULTS DUE the Thursday before your scheduled orientation at 12:00pm for Friday New Hire Orientation
• Contact one of our clinics to schedule your test. Let the clinic know you are an employee at People Incorporated. Bring the TB Test Form with you and make sure you take a copy of your results when your test is done! Submit the completed results to Human Resources.
Ø THIRD: Complete the pre-employment forms and trainings
TIME SENSITIVE All pre-employment forms and trainings are due the Tuesday before your scheduled orientation at 5:00pm
•Complete the HIPAA training and Knowledge Assessment with a score of 70% or better. Sign in as a guest for the Knowledge Assessment. Print or save the score page and submit it to Human Resources.
•Complete the Vulnerable Adult/Mandated Reporter training and assessment. Submit the certificate of completion to Human Resources.
•Complete the Fraud, Waste, and Abuse training and assessment. Submit the signed attestation to Human Resources.
•Employee Information Form
• Form W-4
•Review the Hepatitis B Fact Sheet and then complete and submit the Hepatitis B Consent Form
• Provide us with proof of your highest level of education completed. Degrees beyond a high school diploma require submission of a transcript. The transcript may be a copy.
•Complete one SEXUAL EXPLOITATION BACKGROUND CHECK FORM for each employer where you worked in direct client services over the last five years. Submit the form(s) to People Incorporated and we will mail them out and track receipt.
• If driving is a requirement for your position please submit:
•o A copy of your valid driver's license
•o Proof of current auto insurance.
