People Incorporated’s Adult Mental Health Urgent Care is searching for a full-time In Reach Case Manager 211IRCM. This program offers crisis stabilization services to individuals 18 years old and older residing in Ramsey, Dakota or Washington counties. These services are a collaborative effort between People Incorporated and Ramsey County. Services include walk-in assessment, onsite psychiatric care and mobile crisis visits. Short term therapy and assistance identifying necessary resources may be provided for those transitioning from hospitalization.
Typical hours for this position are Monday – Friday, 9-5 with some variance.

| Duties: |
This role will work with the emergency departments at Regions, St. Joes, and United Hospitals. Services provided include short-term, intensive case management designed to assist a client in accessing financial, medical and social services with the goal of reducing unnecessary visits to the hospital emergency room.

| Qualifications: |
Required Qualifications:
- Bachelor’s Degree in Human Services
- One year experience in case management
Preferred Qualifications:
- Master’s Degree in Human Services
