| Description: | PT (M - F 12:00 pm - 5:00 pm) Outreach Case Manager position available at a Metro area homeless services program. |
| Duties: | The purpose of this position is to engage and support chronically homeless adults who have chemical dependency issues. Outreach Case Managers work out of the programs drop-in center and are expected to both cover drop-in hours and go out into the community where unsheltered homeless adults live. Case Managers are expected to be familiar and comfortable with Harm Reduction and 12 Step recovery philosophies. Basic services provided by Outreach Case Managers include but are not limited to material support for unsheltered living, assistance accessing benefits, assistance accessing physical, mental and chemical health care, assistance with transportation, supportive advocacy and encouragement, and referrals to programs that provide housing subsidies, medical care, and legal aid. Part Time Outreach Case Managers are expected to engage with and provide flexible case management to 25 program participants. The Outreach Case Manager works with each of these participants to develop realistic visions of Mental Illness/Chemical Dependency recovery by helping them identify, develop, and work toward their individual goals. |
| Qualifications: | Bachelor�s Degree in Social Services Field or equivalent life experience and SIGNIFICANT DIRECT WORK EXPERIENCE WITH INDIVIDUALS WHO HAVE EXPERIENCED HOMELESSNESS AND MENTAL ILLNESS required. Master's Degree in Social Services Field, and fluency in Spanish desired. |
